As our practice continues to grow, it is best to call the office for the current status of each physician’s availability.
Please come in 10 minutes before your first appointment to complete forms and set up your account information. Please bring in a current health insurance card, as well as a photo ID (a driver’s license for the parent or guardian is acceptable).
We participate with most major health care plans including Excellus, MVP, Cigna, Aetna, and Medicaid. As participating providers, we agree to submit charges to these insurances for reimbursement.
Please remember that medical insurance is not a substitute for payment. Some insurance companies pay fixed allowances for certain procedures, others pay a percentage of the charge, and still others apply the charges to a deductible plan, reimbursing after deductible limits are met. It is your responsibility to pay any deductible balance, co-pay, co-insurance or any other balance not covered or paid for by your health insurance. Payment is expected at the time of service for all charges and co-pays not pursuant to insurance submission.
